Community Health Network, Inc. (“Community”) has announced a data security incident involving unauthorized access to an employee email account containing patient information. According to the healthcare provider, a malicious actor accessed the account between February 25-26, 2025, prompting an internal investigation supported by outside cybersecurity professionals.
The investigation determined on May 8, 2025, that the affected account contained protected health information (PHI) belonging to certain Community patients. On July 15, 2025, Community confirmed the types of data that may have been exposed and began preparing notifications.
Letters began mailing to potentially affected individuals on September 12, 2025.

The compromised account reportedly contained patient names, medical information and health insurance details. Community stated it has no indication that the information has been misused, but the nature of the data involved presents significant privacy concerns. Secondary reports suggest that approximately 13,939 individuals were affected by this incident.
